Within two weeks: an itemized settlement and a check for the full purchase amount.
Before you drop off
Every donation needs two things.
School/group.
A parent or guardian's signature.
Every donation must include the Izzi's Gym permission form signed by a parent or guardian. Paper and digital options are provided.
